For out-processing, please provide a copy of your orders to the Family Health Check-in desk no early than 10 days before your final out. Medical records will be forwarded to gaining unit upon the receipt of orders. For new patient registration, please call prior to booking any medical appointments.

Release of Information provides copies of medical records to members, legal, and insurance companies as requested. Record requests may take up to 30 days to complete.
- For active duty personnel separating or retiring you will receive both a paper copy and a disc.
- First copy of records is free; additional copies are $0.13 per page with a cap of $75.
- To initiate a request, please fill out a Release of Information form, DD Form 2870 and turn into the medical records department.
